Abstract: In response to the increased quality assurance and improvement requirement in health care sector, at least five hospitals have implementing "5-S practice" to improve their working environment, operations management and minimize the daily risk. Apparently, no systematic review on the implementation of 5-S practice on laboratory work of a medical institution has been conducted since its implementation in hospital since 1998. In this project, the impact of implementing 5-S practice on laboratory work was studied with the use of action research method in a medical institution. With the use of action research, samples of 28 laboratory staff in a medical institution were interviewed to identify the problematic situations in laboratory. Data collected from the interviews were analyzed. Together with the unobtrusive method and observation, problems of unsatisfactory workplace environment, low operational efficiency, lack of laboratory safety management and low staff morale was identified and observed. Actions were followed, these included enhanced 5-S training, top management and laboratory staff commitment and implementing 5-S practice were then taken to solve these problems. Evaluation of the actions performed was conducted through second data collection by observation, unobtrusive method and interview on the laboratory staff. The outcomes of the evaluation revealed that the actions were effective. The laboratory works were improved with better working environment (% satisfaction increased by 46%), enhanced efficiency (% satisfaction increased by 43%), improved laboratory safety management (% satisfaction increased by 32%) and higher staff morale (% satisfaction increased by 32%). Other areas for improving the performance of laboratory were identified in regard to the results of evaluation. These were the provision of education/training programs to the laboratory staff for the continuity of the 5-S practice, analyzing the underlying cause of the persistent unsatisfactory conditions in laboratory, tight management control on the 5-S activities and establishment of appraisal system for the 5-S practice. The study might contribute to the other department in the institution and may also be useful for other hospitals various department in order to solve those problems or situation of tidiness/ cleanliness, inefficiency, low morale etc.
