| dcterms.abstract | The purpose of this study is to validate the Modified Lawton Instrumental Activities of Daily Living (IADL) Scale for the assessment of elderly with disabilities. The scale was modified from the original Lawton IADL Scale (Lawton and Brody, 1969) by the working group of occupational therapist of the Hospital Authority in 1999. The Lawton IADL Scale is commonly chosen as a standardized measure in clinical assessment of instrumental activities of daily living as it is simple, fast and easy to administered (Chan at al., 1998). Prior to the present study, the content validity has been established by the working group. This study aims to further evaluate the construct validity, inter-rater reliability, test-retest reliability and internal consistency of the instrument. A total of 266 elderly persons, with age ranging from 65 to 97, were recruited from the out-patients and in-patients of Kwong Wah Hositpal, who are living independently in the community, and also the residents of a care and attention home. Through factor analysis, one factor is extracted from the scale, which confirmed that all the items are measuring one and the same construct of IADL. Moreover, there is significant difference between the total IADL scores of the two groups in known groups method (t = 7.825, p < 0.01). The results showed that the Lawton's IADL scale could residential differentiate the clients who are living independently and others who need residential support. This result contributes to the construct validity of the scale as a measure of IADL ability. Cronbach's Alpha (a = 0.8869) obtained for the evaluation of internal consistency indicated the 8 items of the scale were closely related to each other and the total score, providing support for a good internal consistency. The inter-rater reliability is also high (r = 0.9825) and the test-retest reliability is also considered to be high (r = 0.9689), which indicate that the scale can be administered among different trained assessors and across different occasions without a significant measurement errors. As for clinical application, the Modified Lawton IADL Scale can be used as an outcome measure of rehabilitation program, screening of the IADL of the elderly that enhance early intervention, and prediction of the placements or the service needs of the of the pre-discharged patients, such as return home and live independently, home with supportive services like meals-on-wheels or other home help services, or other residential facilities, because IADL performance is an important indicator addressing whether an elderly can live independently in the community or not. | en_US |
