dcterms.abstractPurpose: The purpose of study is to compare the outcomes between usual care in hospital diabetes management and an intervention program introduced by nurse specialist. The intervention program is believed for facilitating early discharge. The objectives of intervention program include the shortening of the length of stay; the improvement of glycemic control; the cut of the cost of health service, the enhancement of treatment adherence and patient satisfaction. Method: An experimental, controlled trial was employed. Subjects were assigned randomly to experimental group or control group. Experimental group patients received regular follow-up frequently to adjust treatment regime by diabetes nurse specialist after discharge. Control group patients stayed in hospital for stabilization of blood glucose level, meanwhile patients were discharged upon with reasonable blood glucose level. Both groups would be followed up in medical clinic as usual. Results: The results showed that experimental group had shortened the length of stay; improved glycemic control; enhanced exercise adherence and self-monitoring blood glucose monitoring adherence; but there was no difference on the patient satisfaction between two subject groups. The findings of this study indicate the positive impact of early discharge program to patients and health care professionals. In experimental group, patients can improve self-care adherence and achieve desirable glycemic control through the transitional care from hospital to community. This study can demonstrate the holistic approach of diabetic nursing care and the cost containment of diabetic care model by the nurse-led intervention program. Finally, some research questions were identified for further studies. Conclusion: The early discharge program on diabetic patients can continue in study hospital. The protocol can be extended to out patient clinics after modification, so the admission rate of diabetic patients can be reduced. But the number of diabetes nurse needs to be increased for coping with a large amount of diabetic patients, if the intervention program to be incorporated in the long run.en_US
