dc.titleDevelopment and validation of a Chan's constipation assessment form for children with disabilitiesen_US
dcterms.abstractConstipation has been an ongoing problem experienced by children with different kinds of developmental disabilities. The problem is always overlooked in busy clinics in Hong Kong. The habitual use of laxatives, instead of improving the symptom, has ironically aggravated the problem by causing neural damage to the intestinal mucosa. The purpose of this research was to develop and validate an assessment tool, namely Chan's Constipation Assessment (CCA) for constipation in children with developmental disabilities. The assessment form consists of eleven domains with 49 items. This assessment form could also be used for performance evaluation of various approaches for treating constipation as well as various causes that lead to constipation. The validation process in this study included expert panel review, inter-rater and test and re-test reliability. Thirteen experts were invited to review 45 items in CCA for the relevancy of the content. Overall result of the panel review was satisfactory and no items were deleted. A total of 48 parents, who had children with various degrees of developmental disabilities and been suffering from constipation, are recruited for attending a personal interview. Twelve of these parents who joined the inter-rater reliability study were experience on willing to join the test and retest reliability study after two weeks time. Overall, the result of inter-rater reliability was very good. The average ICC measure was above 0.8 except Question 15 (change of stool consistency). The ICC measure was 0.7 in Question 15. Some reconstruction of the question format in Question 15 need to be done to clear misunderstanding between the raters or the parents in the test and retest reliability. The ICC in the test and retest reliability was lower than what we expected especially in the domain of definition of constipation, part of the children's experience on constipation in the area of blood, sensation, and incomplete emptying and oozing. For the rest of the assessment form the ICC ranged from 0.73 to 1.0 and was very satisfactory. Revision of the questionnaire was needed based on this study to improve the validity and reliability of the assessment form.en_US
