| dcterms.abstract | Objective: To compare the outcomes (HbAlc, health care utilization and patient satisfaction) between the routine care in DM clinic and Diabetes Nurse Clinic. The nurse intervention is believed to improve glycaemia, patient satisfaction and health care utilization for the Type 2 elderly patients with diabetes. Design: A Quasi-experimental design with a pre- and post-test Subjects: One hundred and fifty poor glycaemic control elderly patients with diabetes were recruited in the study. They had their fallow-up either in the standard care or diabetes nurse clinic. Results: The study results show an effective model of diabetes nurse clinic in giving consultation and education to the Type 2 patients with diabetes. Subjects in the nurse follow-up group showed an improvement in the HbAlc, patient satisfaction and health care utilization. However, the systolic blood pressure showed a significant improvement in control group only. The diabetes nurse clinic would not be effective without the support of the multi-disciplinary team. Conclusion: The study provided evidence to support the diabetes nurse clinic in treating the elderly patients with diabetes. This study has provided confidence to the diabetes nurse to treat the unstable elderly patients with diabetes. | en_US |
