dcterms.abstractNew rapid methods for highly sensitive and specific detection of vancomycin tolerant pneumococci may be able to improve the diagnosis and management of antimicrobial pneumococcal infection but are also important for antibiotic treatment studies and vaccine development. The purpose of this study was to develop a sensitive and specific PCR method for the detection of vancomycin tolerance in S. pneumoniae and to study the applicability of these methods to detecting vancomycin tolerant type pneumococci from clinical samples. A PCR assay was established for detecting pneumococcal pep 27 DNA, and this pep27 is believed to be absent or in mutated form in most vancomycin tolerant pneumococcal strains. By determination of gene pep27 with specific primers, we were able to rapidly identify vancomycin tolerance in clinical samples and this method could replace the cumbersome cell culture method. The sensitivity and specificity of PCR assay were found to be 100% and 71.4% respectively on the basis of the results of time kill curve method. We conclude that pep 27 PCR may be useful for rapid detection of vancomycin tolerant Streptococcus pneumoniae which could allow for appropriate treatment of seriously ill patients to be commenced promptly.en_US
