The relationship of serum endothelin-1 concentration and elevated blood pressure in Hong Kong Chinese elderly with metabolic syndromeen_US
Introduction: Metabolic syndrome (MetS) is a collection of heart disease risk factors included hypertension, dyslipidemia, elevated fasting blood glucose and central obesity that increase the chance of developing cardiovascular disease, stroke and type II diabetes mellitus. The metabolic syndrome is strongly associated with increased risk of cardiovascular morbidity and mortality. The prevalence of MetS was high in all developed area, especially the Asian countries is growing. The hypertension was the leading component of developing MetS and associated with mortality of its complications. Endothelin-1 (ET-1) is a potent vasoconstrictor that mediates the vascular tone and associated with endothelial dysfunction which is the early stage of MetS. The treatment for hypertensive aged people with MetS were limited, the understanding of pathophysiological vascular effects of ET-1 in hypertension can offer a wide range of pharmacologic options for those people. Aims: The aims of the study were: I) to determine the ET-1 concentration in the Chinese subjects who aged over 60 with and without MetS according to the NCEP ATP III definition. II) to determine the association of the ET-1 concentration and elevated blood pressure in the Chinese diagnosed MetS subjects who aged over 60. Methods: A total of 79 subjects were included in the study. They were divided into control group and MetS group according to the NCEP ATP III definition for MetS diagnosis. 5 parameters (systolic blood pressure, diastolic blood pressure, waist circumference, high density lipoprotein and fasting glucose) were measured. ET-1 was measured by ELISA method. The association of the different components and ET-1 concentration were studied. Results: ET-1 concentration is significantly elevated in MetS group compared with control healthy group. ET-1 concentration is a significantly associated with systolic blood pressure, waist circumference, high density lipoprotein cholesterol and fasting glucose. No association was found in diastolic blood pressure and ET-1 concentration. There was no sex different in the ET-1 concentration in MetS group and control group. Conclusion: ET-1 has a key role in pathogenesis of MetS and hypertension in aged population.en_US
