dcterms.abstractEpithelial ovarian cancer (EOC) is long been acknowledged as one of the deadliest gynaecological malignancy. Its lymphatic involvement beyond ovaries is in parallel to its disease stage. Aggressive spread pattern often seen suggests occult metastasis is evolving when the tumour was seemingly still confined to the ovaries (I). Method of detecting early metastatic event in patients of all disease stages and remission stages therefore become necessary. Overall, there is about 30% of reported rate of upstaging,recurrent rate of stage I disease is 20-25%, and later reach 80% in stage III and IV in literature (2). We propose that high relapse rate and low disease free survival can be improved with a more comprehensive method of screening for occult metastases. The purpose of this study was to introduce serial sectioning and cytokeratin (CK) stained method to screen for occult metastases in the form of isolated tumour cells (lTCs) in the lymph nodes that may have been overlooked by the conventional H&E staining method. We have randomly selected 23 patients and re-examine of their FFP lymph node section found that ITCs can be observed in 27% of previously H&E screened negative sample. CK stain method with serial sectioning technique can generate a positive prognostic indication on upstaging of EOC, an additional safe keeping procedure for screening of metastases alongside with the standard H&E method is recommended when pathologist determining its disease stage.en_US
