Abstract: | 本文旨在深入探讨医生经济激励如何影响患者的医疗服务需求,特别是通过解析医生推荐与患者选择之间的动态过程。过去的研究显示,医生因某种医疗服务的经济利益而倾向推荐时,可能促使患者需求增加,这种需求增加可能并非基于医学最优考量,进而对患者的健康产生负面影响,同时加重医疗系统的经济负担。然而,这些研究主要关注患者最终的服务使用情况,忽视了医生与患者决策互动的细节。本研究通过实证研究,分析了医生和患者在医疗服务决策中的互动,更全面地揭示了需求诱导的机制及其后果。 研究发现,降低医生的经济激励与患者购药概率减少相关,这一结果印证了医生物质利益与医疗服务利用的正相关关系。其次,经济激励的减少不仅影响医生的处方行为,还间接影响了患者的购药决策。进一步的分析表明,经济激励的降低并不存在医生选择依从性更强患者开处方的原因,而是改变了医生与患者沟通的方式,特别是在药物必要性和有效性的说服上,反映在医生的咨询内容更为简洁。另外,在激励减少后并未影响患者对咨询服务的满意度。 本研究不仅深化了对医生经济激励与患者需求之间关系的理解,还为政策制定者提供了有价值的参考,有助于优化医疗系统的激励机制,促进患者健康与医疗成本的平衡。 |
