Medical records of patients with stable hypercapnia COPD who were admitted to the medical department of North District Hospital, Hong Kong between 1 January 2014 and 31 December 2021 were reviewed. | dcterms.abstract | A total of 86 patients were included, of which 68% were male with a mean age of 79 years old. The mean AED visits were significantly lower among home NIV with telemonitoring users compared with nonhome NIV users (p<0.001) at 3 and 12 months (0.7 vs 2.5 times at 3 months, 2.1 vs 6.8 times at 12 months). Hospital readmissions due to exacerbation of chronic obstructive pulmonary disease (AECOPD) were significantly lower among users of home NIV with telemonitoring compared with nonhome NIV users (28 out of 66 versus 17 out of 20 (p<0.001) at 3 months and 51 out of 66 versus 20 out of 20 (p=0.02) at 12 months. The mean time to hospital readmission was significantly prolonged among users of home NIV with telemonitoring compared with non-home NIV users by 46.4 days (84.8 vs 38.4 days) (p=0.05) at 12 months. The mean days of hospital stay were significantly shorter among home NIV with telemonitoring users compared with nonhome NIV users by 11.2 days (5.9 vs 17.1 days) (p=0.01) at 3 months and 25.6 days (19.0 vs 44.6 days) (p<0.01) at 12 months. | en_US |

| dcterms.abstract | This retrospective case-control research highlights the effectiveness of home NIV with telemonitoring, particularly in decreasing AED attendance and minimizing hospital readmissions related to AECOPD at 3 and 12 months. Our study also demonstrated that home NIV with telemonitoring significantly prolong time to hospital readmission at 12-month interval and reducing day of hospitalisation at 3 and 12 months, which represents novel findings in this context. Future research should investigate adherence in conjunction with other respiratory metrics, such as tidal volume, respiratory rate leakage and pressure, to provide a thorough assessment of home NIV with telemonitoring. | en_US |
