Abstract: The problem of discharging older patients successfully into the community is a great concern of health care professionals and the government. Thirty four older patients who had been discharged home were followed up between the period of July 1998 and January 1999 to learn about their post-hospital needs. Carers of the subjects were also recruited to assess their level of stress in caring for, and the support they provided to, the older people. Sixteen subjects were readmitted through the emergency department within three months of discharge. Among the sixteen subjects, five of them died in the hospital. As a result of the findings of the assessment, tentative risk scores for readmission to hospital (or death) were calculated. Total risk scores of individual subjects identified those who were at risk of early readmission. These scores are suggestions only at this stage and are yet to be tested for validity and reliability. Although statistical significance could not shown in this study, the study findings have implications for the development of an organized discharge plan for this group of elderly people. There should be awareness by health care professionals of the health needs of their older patients.
