| Abstract: | Aftercare Service had been development more than ten years in Hong Kong. The service mainly focus on monitoring clients' mental status, the amount and duration of re-hospitalization, employment and other personal problem. The main functions are more concerned with tertiary prevention or rehabilitation. To review a year experience in aftercare service, good informal social support can help the client more quickly and immediately. The support will be more explicit in time of crisis. In this practice project, writer will understand more about the function, the determinants of social support and the relationship between social support and mental illness. Three case studies was used for the study. In-depth interviews were used. The findings have profound implication for aftercare service and worker's intervention. |
