dcterms.abstract | Flexible flat feet is one of the commonest lower limb conditions of children in Hong Kong. Different level of lower limb problems such as, strain, pain, discomfort and other co-morbidity can occurred in patients with symptomatic flexible flat feet. Twenty-one subjects, 15 male and 6 female, between the age of seven and fifteen years were recruited in the study. All subjects were newly diagnosed with symptomatic flexible flat feet and provided with UC-BL foot orthosis as orthotic intervention for ten weeks. American Academy of Orthopaedic Surgeon (AAOS) Lower Limb Outcomes Assessment-Foot and Ankle Core Scale was used to determine the functional status of the subjects before and after the orthotic treatment. Range of structural and functional foot characteristics including, Resting Stance Calcaneal Angle (RSCA), Arch Index (AI), Inter Malleolar Distance (IMD) at RSCA position, Contact Force Ratio (CFR) and plantar loading information were recorded to determine the foot characteristics of the subjects before, during and ten weeks after the first day of treatment. Descriptive information, including gender, Body Mass Index (BMI), age, foot dimensions and Beighton Score together with the foot characteristics were integrated to determine the effect and extended as predicator to determine the functional status of paediatric patient with symptomatic flexible flat feet. The results showed that the UC-BL foot orthosis can improved various aspects of the functional score significantly. On the other hand, the instantaneous change of the RSCA and IMD induced by the UC-BL foot orthosis was the major factors which contributed to the significant improvement of the functional score of the subjects. A greater change of these factors induced a greater extent of functional improvement to the paediatric patient with symptomatic flexible flat foot. Based on the findings, the Resting Stance Calcaneal Angle and the Inter-Malleolar Distance at RSCA position can be used as an indicator for the prescription of UC-BL foot orthosis to paediatric patient with symptomatic flexible flat foot for functional improvement and symptomatic relief. Maximal correction should be provided to the abnormal pronation of the subtalar joint in order to have a greater extent of functional improvement. | en_US |
