dcterms.abstract | The impetus for this project arose simply because there is not any available Chinese language instrument for measuring quality of life (QOL) for nasopharyngeal carcinoma (NPC) patients. Recognizing NPC is a type of head and neck (H&N) cancer, the author attempted to establish a Chinese version of the Quality of Life Radiation Therapy Instrument with Head and Neck Companion Module (QOL-RTI/H&N) through translation and tests. The thesis opens with the justifications and objectives of the study. Chapter Two provides an overview of literature reviews. Chapter Three outlines the principles, designs and methods of the study. The study was divided into three phases: translation, pre-testing, and field testing. The methods, procedures and results of the translation are described in Chapter Four. The modified double-back translation method was used. Apart from adapting existing translation frameworks, certain strategies were added in so as to ensure that the translated version is equivalent to the original form. Key features of the methodology in translation were the involvement of patients and original developers of the QOI-RTI/H&N. Pre-testing the instrument helped to identify any previously undetected difficulties, hence facilitated the implementation of field testing. In this stage, the author employed both qualitative and quantitative approach to examine cross-cultural equivalence, applicability and acceptability of the questionnaire. Qualitative approach using focus group and individual semi-structured interview confirmed the comprehensibility and acceptance of the questionnaire by local target patients. Pilot testing of the questionnaire had given the hints for planning psychometric evaluation in field testing. Two samples of eligible subjects were recruited for field testing while they were waiting for radiotherapy. In total, the questionnaire was completed by 159 patients with mean age of 50 years. Internal consistency was satisfactory for all subscales except socioeconomic dimension. Cronbach's alpha of the overall QOL-RTI/H&N using Sample 1 was 0.85 and Sample 2 was 0.93. Test-retest reliability yielded an intraclass correlation coefficient (ICC) of 0.71. In validating concurrent validity, Pearson correlation coefficient showed a fairly strong correlation between the QOL-RTl/H&N and the Chinese FACT-G (r = 0.82, P < 0.001). In the examination of responsiveness to change, paired t test showed significant results on most of the items when comparing the data at the sixth week of the treatment with the baseline. A large effect size of 3.21 implied that there was a great decline in the QOL due to the side effect of treatment. On the whole, most of the findings were comparable to those reported in the US study. Patient compliance was high (90.7%). The Chinese QOL-RTI/H&N could he useful for local research and clinical practice. | en_US |
