dcterms.abstractAim: The aim of this study was to examine the effect of a nurse-led nutritional program on minimizing the nutritional problems of head and neck cancer patients undergoing radical radiation therapy as indicated by the percentage weight loss, quality of life, performance status and the frequency of hospitalization due to dehydration. Design: The study was conducted in the oncology department of a public hospital in Hong Kong. Subjects without history of psychiatric illness, end stage renal and liver disease and secondary malignancy were recruited. Sixteen eligible subjects (i.e. the intervention group) were followed up by the researcher weekly from the third week till the end of their radiation treatment. The nurse-led nutritional program was implemented. Their body weights were measured before their treatment started (T0) and then weekly from the third week (T1) till the end of their treatment (T2). Their quality of life and performance status were assessed by using the Hong Kong Chinese version of EORTC QLQ-C30 (Version 3) and EORTC QLQ-H&N35 and ECOG performance status scale before the commencement of their radiation therapy (T0), at week three of their treatment (T1) and at the last week of their treatment (T2). For the control group, medical records of 35 subjects (i.e. the control group) who had completed their radiation therapy and received weekly physician follow up between 2008 and 2010 were reviewed. Their body weights at T0, T1 and T2 and the incidence of hospitalization were retrieved. The percentage weight change at T1 and T2 and the incidence of hospitalization due to dehydration between two groups were compared. The quality of life and performance status of the subjects in the intervention group at three different time points were compared. Results: Significant difference in percentage weight loss was found between the control and intervention group. The subjects in the control group had more weight loss than those in the intervention group at T1 and T2. The quality of life and performance status of subjects in the intervention group were significantly worsened during their course of radiation treatment. For the incidence of hospitalization, no significant difference was identified. Conclusion: The nurse-led nutritional program can reduce the percentage of weight loss of head and neck cancer patients undergoing radical radiation therapy. It can be incorporated into the care of the head and neck cancer patients undergoing radical radiation treatment.en_US
