Abstract: | 医患关系管理是一个世界性的问题。本研究在回顾前人的发现和理论基础上,根据医患关系、辱虐管理理论和社会交换理论,从患者负向情绪和患者对医生的信任视角,对医生辱虐患者行为与患者就医偏差行为间的相互影响及其内在作用机制进行了探讨研究。通过对浙江省三家综合性公立医院532份医生与患者配对的问卷实证调查,结果表明: 1.医生辱虐行为正向影响患者人际导向(b = 0.682, P < 0.001)和组织导向(b = 0.669, P < 0.001)二个维的就医偏差行为; 2.患者负性情绪对医生辱虐行为与人际导向(b = 0.084, P < 0.01)和组织导向(b = 0.099, P < 0.001)二个维的就医偏差行为均具有部分中介作用。3.患者对医生的信任对患者自己负性情绪与患者人际导向的偏差行为关系具负向调节作用(b = -0.086, P < 0.001) ;当患者对医生的信任高时,患者负性情绪对人际导向的偏差行为的影响不显著(简单斜率b = 0.051, P > 0.1) ;但当患者对医生的信任低时,患者负性情绪对人际导向的偏差行为具有显著的正向影响(简单斜率b = 0.185, P < 0.001) 4.患者对医生信任对患者自己负性情绪与患者组织导向的偏差行为关系具负向调节作用(b = -0.064, P < 0.01) ;不管患者对医生的信任高(简单斜率b = 0.091, P < 0.05)或低(简单斜率b = 0.193, P < 0.001)时,患者负性情绪对组织导向的偏差行为均具有显著的正向影响。本研究的理论贡献在于通过实证方法,运用来自企业的辱虐管理理论在医疗情景等的应用,对医生辱虐行为与患偏差行为的影响及其内在作用机制,进行了组织行为学实证研究;本研究的实践意义在于将辱虐管理理论和本研究成果引入医院管理实践中,有助于从领导学的新视角提高医生对患者的管理水平,从提高医生领导力着行改善医患关系。 |
