|Abstract:||Introduction: The release of endogenous melatonin declines with age. However, melatonin is an easily accessible dietary supplement for older adults with sleeping difficulties. Dual-tasking is a common daily activity that often requires extra attentional resources, especially in older adults. Falling downstairs is a leading cause of accidental death. Studies of the effect of melatonin on postural control after stepping down while performing a concurrent auditory response task are limited; therefore, this study was performed to investigate postural control after stepping down while dual-tasking with or without the administration of melatonin. Method: A randomized, double-blind, placebo-controlled, cross-over study was conducted in 34 older adults, who performed (1) an auditory Stroop test, (2) a stepping-down task, (3) a stepping-down task with a concurrent auditory response, and (4) a 30-second double-leg stance on a foam rubber mat with the eyes open, both before and 1 hour after taking either melatonin or a placebo (randomly assigned). A 7-day washout period was provided between assessment occasions. A two-way repeated-measures analysis of variance was conducted to analyze the interactive effects of treatment and time. Results: A statistically significant difference was found in the sway area before and 1 hour after melatonin administration while dual-tasking and in the 30-second double-leg stance with eyes open. However, no statistically significant difference was seen in cognitive task performance. Discussion and Conclusions: Melatonin decreases postural control in older adults when dual-tasking and when performing a 30-second double-leg stance with eyes open. Because melatonin is easily accessible, safety precautions should be taken with those who have a risk of postural imbalance and are prone to falls.|
